The assessment is free and available for those ages 60 and older

Sanford Center for Aging is introducing the Sanford Assessment, a community-based health and wellness assessment that will provide a comprehensive review with recommendations for participants.

This new assessment replaces a previously offered program but is now provided to participants free of charge and is no longer subject to insurance coverage limitations. The Sanford Assessment is designed to empower participants to be their strongest advocates with:

  • Comprehensive medication review.
  • Screening for risk of falls.
  • Cognition screening.
  • Support for depression, anxiety, isolation and other social risk factors.
  • Personalized supportive services plan (shared with your primary care provider, if appropriate).
  • Referrals to appropriate community resources.

“Our Sanford Assessment is a vital resource for older adults, offering a comprehensive review with our assessment team featuring a social worker and geriatric pharmacist,” said Peter Reed, Ph.D., MPH, director of the Sanford Center for Aging. “We look forward to continuing to serve the community by offering this valuable service.”

In addition to the assessment, Sanford Center for Aging offers other free services to older adults, including evidence-based health and wellness programs, transportation, the Senior Outreach Services companionship program, medication review and education for healthcare providers and caregivers.
