Financial aid forms
The Office of Financial Aid is actively transitioning from PDF forms to DocuSign PowerForms. DocuSign is an application designed to collect and manage digital signatures for forms. PowerForms allow students to complete forms and circulate for signatures 100% digitally.

The V-1 Dependent Verification Worksheet is for dependent students (based on FAFSA criteria, not taxes) who are selected as part of the U.S. Department of Education’s V-1 tracking group. In order to qualify for federal and other need-based financial aid, these students must submit the worksheet and all applicable tax data as outlined within the worksheet.
The V-1 Independent Verification Worksheet is for independent students (based on FAFSA criteria, not taxes) who are selected as part of the U.S. Department of Education’s V-1 tracking group. In order to qualify for federal and other need-based financial aid, these students must submit the worksheet and all applicable tax data as outlined within the worksheet.
The V-4 Verification Worksheet is for students who are selected as part of the U.S. Department of Education’s V-4 tracking group. In order to qualify for federal and other need-based financial aid, these students must submit the worksheet, which includes a statement of educational purpose that must be signed in the presence of a Financial Aid staff member or notary.

The Verification of Amended Taxes form is required of students or parents when the Financial Aid Office is notified that tax information submitted for the FAFSA has been amended. In order to qualify for federal or other need-based financial aid, these students must submit the form and all applicable tax documents as outlined within the form.
The Verification of Non-Filing Statement is required when, as part of a student’s verification process, an IRS Verification of Non-Filing Letter is requested but unable to be obtained.
The Appeal for Independent Status is available for dependent students (based on FAFSA criteria, not taxes) who believe they should be classified as independent. In order to be approved, students must be able to demonstrate that it is either impossible to obtain parent financial information or that it is unsafe to do so.
The Budget Revision Request allows students to request an increase to their Cost of Attendance budget (COA) for circumstances not reflected in the information provided when initially completing the FAFSA, such as out-of-pocket healthcare costs or childcare expenses.
The Citizenship/Immigration Affidavit is available for students who must submit United States citizenship/nationality documents to the Financial Aid Office but are unable to appear in person.
The Consortium Agreement allows students to request that enrollment at another Title IV-eligible school be counted toward their financial aid eligibility at the University.
The Contractual Agreement allows students to request that enrollment at a Title IV-ineligible school be counted toward their financial aid eligibility at the University. This is most commonly used for study abroad programs.
The Degree Applicability Certification is available for academic advisors to notify the Financial Aid Office of degree applicable coursework that a student plans to take but is not already included in the student’s Academic Advisement Report in MyÂ鶹ӳ».
The Dependent Students Without Parental Support (Unsubsidized Loan Only) Form is for Dependent students whose parent(s) are refusing to provide parent information for the student’s FAFSA application. This student will then only be eligible for Unsubsidized Loans.
The Federal Direct Loan Activation allows students to accept loans that have been posted to their MyÂ鶹ӳ» accounts or request an increase to a previously accepted amount up to their maximum eligibility. Students who previously declined federal loans in MyÂ鶹ӳ» and would now like to accept them, and students who experience technical issues accepting loans in MyÂ鶹ӳ» may also submit a Federal Direct Loan Activation Form.
The Federal Funds Return form is for students who have received federal loan funding but would like to return some or all of it within 14 calendar days of the loan disbursement.
The Financial Aid Cancelation Request allows students to cancel financial aid for the current semester or school year. If you would like to cancel federal direct loans that have already disbursed, you must submit the Federal Funds Return form instead.
The Funding Level Change Request is required when students are changing enrollment level (for example, full-time to half-time) after financial aid has been disbursed. Please note that changes to enrollment may reduce financial aid eligibility, leading to required repayment of funds.
The Institutional Methodology for FAFSA-Ineligible Students allows students who are not eligible to complete a FAFSA (DACA students, undocumented students, international students, etc.) to request an evaluation of financial need. Depending on the outcome of the financial need evaluation, FAFSA-ineligible students may be offered aid from non-federal sources, such as institutional grants and scholarships.
The Maximum Time Frame Appeal is available for students who will be unable to complete their degree within 150% of the average length of their program to appeal for continued financial aid eligibility. The Physician’s Certification and Borrower’s Acknowledgement of Obligation is available for students who have had previous federal loans discharged, as a result of Total and Permanent Disability, to request that their eligibility for future Pell Grant and/or federal loan funding be reinstated.
The Re-Evaluation of Income/Benefits form is available for students who have experienced significant changes to personal or family income in a tax year after the year of tax data required by the FAFSA. Approval may result in changes to a student’s Expected Family Contribution (EFC), Cost of Attendance budget (COA) or both.
The Reporting Changes form allows students to correct information regarding their living arrangements or notify the Financial Aid Office of resources not currently included on their most recent financial aid offer email.
The Satisfactory Academic Progress Appeal is available for students who have fallen below financial aid GPA or pace (pass rate) requirements to appeal for continued financial aid eligibility. The Scholarship Appeal/Deferral Request allows students who have already been awarded scholarships to appeal lost eligibility or request scholarship deferral. There is no appeal to be considered for scholarships that have not already been awarded and no exceptions will be made for missing application deadlines. Do not submit this form if your appeal is regarding the Governor Guinn Millennium Scholarship - the Â鶹ӳ» Treasurer's Office does not accept appeals.
The Scholarship/Fellowship Request allows academic or administrative departments to submit funding information on behalf of students for whom they would like to provide scholarships or fellowships using departmental or department-managed funds.
The Special Circumstances form allows students to request special consideration for circumstances not reflected in the information provided when initially completing the FAFSA, such as divorce or loss of child support. Approval may result in changes to a student’s Expected Family Contribution (EFC), Cost of Attendance budget (COA) or both.
The Student Release for Third Party Agencies allows the Financial Aid Office to share financial aid information with organizations not affiliated with the University. Most commonly, this is for students who are receiving external scholarships and must share financial aid information with those organizations for eligibility purposes.
The Study Abroad Authorization Request is a required form for all students wanting to utilize their financial aid & scholarship resources while studying abroad. This form is for USAC specialty, USAC partnership, OISS Exchange, and other abroad programs. Please note that this form is a request form and will result in notification of financial aid eligibility for your desired study abroad program.
The TEACH Grant Application allows students who plan to teach in high-need fields in schools to apply for Teacher Education Assistance for College and Higher Education (TEACH) Grant funds. Degree program and work obligations apply – details are provided on the form.
The Thirty to Complete Plan of Study allows students to request exceptions to enrollment requirements for some types of financial aid, such as institutional grants and scholarships. There are no exceptions to enrollment requirements for federal grants and loans.
The Transfer Monitor Clearance Statement is for students who have been identified by the U.S. Department of Education as having financial aid scheduled to be disbursed at another school during the same semester that they are scheduled to receive aid at the University. Students must submit the form and ensure that aid at all other schools is canceled to remain eligible for financial aid at University.
The Unusual Enrollment History form is for students who have been identified by the U.S. Department of Education as having received federal aid at multiple institutions within the last four award years. Selected students must submit the form to the Financial Aid Office before federal aid eligibility can be determined.
